How to Refer

Referrals in Western Australia

Regional Health Link accepts referrals from public and private hospitals, private health insurers, and clinical services across Western Australia requiring clinically supervised patient transport.


Our referral process is designed to support regional, rural, and metropolitan transfers, ensuring continuity of care across long distances and variable service environments.

Who Can Refer

 We accept referrals from:


  • WA public hospitals and health services 
  • Private hospitals and day procedures centres
  • Emergency departments
  • Medical practitioners and specialists
  • Nursing and allied health teams
  • Private health insurers coordinating patient transfers
  • Services supporting regional and remote patient access


If referral suitability is uncertain, our team is available to discuss the patient’s clinical and transport needs prior to transfer.

When to Refer

 Referral to Regional Health Link may be appropriate for patients who:


  • Require clinical oversight during transport
  • Require advanced care and oversight during transport
  • Urgent or time-critical inter-facility transfers 
  • Are being transferred between facilities within WA
  • Require long-distance road transport from regional or remote locations
  • Are unsuitable for routine non-clinical transport
  • Require a clinical escort or monitored transport environment
  • Are accessing specialist or hospital-based care unavailable locally
     

Each referral is reviewed with consideration of patient condition, clinical risk, distance, and service availability.

Information Required for Referral

To support timely assessment and coordination, the following information is requested where available:


  • Patient details
  • Clinical summary and current observations
  • Relevant medical history or identified risks
  • Referring service and contact detail 
  • Receiving service and destination
  • Transfer timeframe and urgency 
  • Any relevant documentation or discharge information
     

Incomplete referrals can still be discussed — our team can guide next steps.

How to Make a Referral

Further documentation or referral forms will be requested if required following initial contact.

What Happens Next

 Once a referral is received:


  1. Clinical and transport requirements are reviewed
  2. Appropriate level of clinical support is confirmed
  3. Coordination occurs with referring and receiving service 
  4. Transfer is undertaken with continuity of care maintained through to handover
